Tropical Cyclone Eline struck Mozambique in 2000, bringing weeks of heavy rainfall and devastating floods. Blaise Judja-Sato visited the country at the request of Graça Machel to assess how introducing satellite telephones could help, finding destroyed infrastructure and supply chains that prevented many communities from accessing essential health services.

In response to a critical lack of fuel to support the cold chain in Mozambique, VillageReach and Fundacao para o Desenvolvimento da Comunidade (FDC) established VidaGas, a Mozambique for-profit propane distribution company with a social mission: to bring clean-energy solution to rural health clinics and create a sustainable business model that improves health.

After the first five years of implementation of the Dedicated Logistics System for Vaccine Service Delivery, vaccine coverage rates for children 24-35 months old increased from 68.9% to 95.4%, and reports of vaccine stock outs by rural health centers decreased from 80% to 1%. Over time, these supply chain improvements also proved to be 17% more cost-effective for the Mozambique Government.

The groundbreaking five-year project in Cabo Delgado to strengthen the country’s vaccine delivery system and increase Access to Health Products, addressed cold chain logistics, supply stock-outs and staff support, dramatically improving immunization rates and health service access for 1.5 million people in this remote province. This pioneering initiative laid the foundation for VillageReach’s scale-up, marking a critical moment in the organization’s journey to extend quality health care to everyone.

VillageReach began working in Malawi when the country’s infant mortality rate exceeded 70 deaths per 1,000 live births. Today, that number has dropped below 30.The Kwitanda Community Health Project strengthens rural health Service Delivery through community-led initiatives, including training local health workers and improving maternity services.

Growing from a single district, Balaka, a focus on nutrition and maternal, newborn and child health, Chipatala cha pa Foni strengthens Service Delivery by providing free information provides free information on a wide range of health and nutrition topics to all Malawians 24 hours a day, 7 days a week, and referrals to care. CCPF is one of the first government-run, nation-wide health hotlines in Africa.

The Pharmacy Assistant Training Program addressed Human Resources for Health and shortages of pharmacy personnel in rural Malawi by offering a two-year certificate program combining classroom instruction and practicums. The program has led to an 80% reduction in clinician time spent on burdensome logistical tasks, enhanced data reporting accuracy and better drug management standards. Over 2000 Malawi pharmacy assistants have been trained via this initiative.

VillageReach partners with ministries of health in Mozambique, Tanzania, and Zambia to pilot Open Logistics Management Information System (OpenLMIS), an open-source eLMIS platform. From that initial launch, the system expands to eight countries—including Benin, Côte d’Ivoire, Zanzibar, Malawi and Angola—enabling real-time logistics, reducing stockouts from 19% to 5%, and cutting data delays from one month to one day. VillageReach continues to assist private sector providers in platform development and peer-to-peer learning, cementing OpenLMIS as a flagship tool for resilient, data-driven health supply chains across Africa.

Chipatala Cha Pa Foni expanded rapidly toward national scale, and VillageReach emerged as a global leader in practical drone applications for medical deliveries. This validated VillageReach’s scalable, last mile health system innovations.

VillageReach expanded to Democratic Republic of Congo (DRC) to increase Access to Health Products and to conduct a supply chain assessment in remote Equateur Province, where some health workers travel up to 80 kilometers to access basic medicines. We found long delays, empty shelves and broken cold chain equipment. This “reality check” helped catalyze the Next Generation of Supply Chain (NGCA) approach, proving that an effective health supply chain can be built to reach the last mile.

VillageReach secures Determined, Resilient, Empowered, AIDS-free, Mentored and Safe (DREAMS) Innovation Challenge funding to support improved Service Delivery with a focus on adolescent health care in districts with high unplanned pregnancy and sexually transmitted infections. Then, the organization is included in the Organized Network of Services for Everyone’s (ONSE) Health consortium for CCPF scale up and supply chain improvement at the district and community levels.

Drone transport harnesses unmanned aerial vehicles to increase Access to Health Products and improve delivery of vaccines and medical supplies in hard-to-reach areas. In 2016, it supported Malawi’s first drone flights focused on cost analysis and data collection. In 2019, VillageReach partnered with the DRC government to launch regular drone deliveries of vaccines and essential health products. By 2022, it piloted drone deliveries in Mozambique to expand access. Drone delivery addresses supply chain gaps, reduces delivery times and strengthens last-mile health services.

VillageReach launches the Next Generation Supply Chain (NGCA) to improve Access to Health Products by organizing the first direct distribution of vaccines and other health products in Equateur province, DRC. NGCA reduces vaccine stock-outs observed in NGCA-supported health districts, increases vaccine doses administered and reduces transportation costs. Over the next eight years, NGCA expands to 13 provinces, all of which are now within the top 15 provinces for vaccine availability across the country.

VillageReach orchestrates change and technical support to ensure smooth rollout by the Expanded Program on Immunization. This strategic collaboration improves immunization coverage in low-access districts, leverages private-sector tech, and sets the stage for national scale-up of mobile-driven vaccine delivery.

VillageReach partners with the Government of Mozambique and USAID to improve Access to Health Products and implement the Last Mile Supply Chain (LMSC) program in two districts in Zambézia province. Over time, LMSC is expanded to support all districts across 10 of Mozambique’s 11 provinces, delivering health products to more than 1,500 health facilities every month, 99% distributed according to plan. LMSC has been transitioned to full ownership and control of the Mozambique government.

VillageReach partners with the Liberian Ministry of Health (MoH) and Last Mile Health to improve Access to Health Products and launch the Supply Chain for Community Health Workers (CHWs) program in Liberia, strengthening last-mile delivery of essential health products. A standardized kitting system streamlines the packaging and distribution of medicines and supplies to CHWs, improving efficiency and product availability. The program’s success contributes to national recognition of CHWs as vital actors within the health system and the integration of CHWs into national supply chain planning and policy.

VillageReach rose to meet the global COVID-19 crisis, protecting health workers and sustaining essential services across Africa. As the pandemic spread, VillageReach leveraged its Health Center by Phone service for rapid outreach and delivered PPE items across 20 countries. Despite global disruption, VillageReach continued delivering critical vaccines and medicines—reaffirming its role as a trusted partner in resilient, equitable health care delivery.

Recognizing that long-term impact depends on strong local ownership, VillageReach works closely with governments to co-create programs that align with national priorities and build capacity for autonomous management. This approach moves beyond traditional exit strategies by emphasizing system strengthening, resource alignment and stakeholder engagement. By 2020, both Kwitanda Community Health and CCPF were transitioned successfully, to a local impact organization and to government respectively—ensuring that these essential health services continue after donor support ends.

VillageReach helped transform Service Delivery with AlôVida into Mozambique’s national health helpline—an important advancement in the country’s digital health system. Originally launched by Fundo Para o Desenvolvimento da Comunidade (FDC) as an HIV-focused hotline, AlôVida was expanded under the Ministry of Health to cover a wide range of health topics. VillageReach provided technical support to integrate Interactive Voice Response (IVR), WhatsApp messaging and a real-time dashboard. These enhancements improved access to reliable health information, supported vaccine tracking and enabled early outbreak detection, making AlôVida a cornerstone of public health.

VillageReach helps to create and joins the COVID-19 Action Fund for Africa (CAF-Africa) and COVAX, successfully suppling Personal Protective Equipment (PPE) and vaccines across our core and partner countries. CAF-Africa committed over 57 million pieces of PPE to protect CHWs in 12 African countries. Relationships made are sustained in coalitions such as Pandemic Action Network.

In the fight against the COVID-19 pandemic, VillageReach transforms Service Delivery and opens Central Africa’s first vaccinodrome to expand access to vaccination, contributing to 33% of the total vaccinated population in Kinshasa by the end of the project. Further vaccination sites are operated in Youpougon, Cote d’Ivoire. An outreach-focused approach increased vaccine uptake.

VillageReach began improving Access to Lab Services through a laboratory sample transport program supporting polio eradication efforts across 15 African countries. This marked a new frontier for the organization, traditionally focused on delivering health products, by tackling the reverse logistics challenge of safely and quickly moving biological samples from communities to testing laboratories. Working with governments and partners, VillageReach applied its logistics knowledge to design efficient, outsourced transport systems tailored for lab samples. This innovation strengthens disease surveillance and rapid response capacity.

The Outsourced Transport Resource Center (OTRC) enhances Access to Health Products across Africa by leveraging private sector logistics. Building on successful models in countries like Mozambique, Nigeria and the Democratic Republic of Congo, OTRC provides governments and partners with tools, training and technical support to implement outsourced transport solutions. Its services include contracting third- and fourth-party logistics providers. By promoting multimodal transport—including drones—OTRC improves the availability of health products in remote areas, contributing to more resilient and equitable health care systems.

VillageReach embeds Responsive Primary Health Care (RPHC) principles across its programs by prioritizing community voices, data-driven feedback and integrated Service Delivery. In Mozambique, the organization expands Bate-Papo Vacina!—its first participatory research and design project—to understand vaccine perceptions. This approach broadens to Malawi and then to a four-country immunization study. In the DRC, RPHC guided expanded Next Generation Supply Chain improves vaccination strategies by combining service delivery, community engagement and the Community Insights to Action feedback loop—reaching over 17,000 zero-dose and under-vaccinated children in remote areas.

VillageReach has partnered with Mozambique’s Ministry of Health to strengthen the country’s Resilience and ability to manage public health crises through the development of Public Health Emergency Operations Centers (PHEOCs). Key milestones include creating the legal and institutional frameworks for establishing a national PHEOC and the successful launch of the first provincial PHEOC in Tete Province. VillageReach has provided technical assistance in infrastructure planning, operations and training. These efforts create a coordinated emergency response system from the national level down to the provinces.

In Kenya, the Community Health Units for Universal Health Coverage (CHU4UHC) initiative empowers Community Health Workers (CHWs) with the training, tools and support needed to deliver life-saving care. VillageReach and Lwala Community Alliance partner with the Ministry of Health to strengthen last-mile supply chains, equip CHWs with essential health kits and roll out digital systems like the electronic Community Health Information System (eCHIS). This work ensures CHWs can screen, treat and refer patients within their communities, bridging gaps in access and advancing universal health coverage nationwide.

The Global Telehealth Community of Practice (GTCoP) advances equitable access to health care through telehealth. This global coalition addresses the challenges of fragmented digital health efforts by fostering collaboration among policymakers, health institutions and technology experts in both Service Delivery and Health Information Systems. Structured around country-level chapters and guided by a strategic Steering Committee, the GTCoP promotes shared learning, policy development and implementation support. Aligned with WHO’s Global Initiative on Digital Health, the GTCoP aims to build robust telehealth ecosystems that reduce access barriers and improve health outcomes worldwide.

In 2024, VillageReach and the Institut National de Préparation Professionnelle (INPP) launch the Congo Drone Académie—the Democratic Republic of Congo’s first drone pilot training center. This marks a turning point for local capacity building in drone technology, addressing the early challenge of relying on foreign pilots for VillageReach’s Drones for Health program. The academy, supported by Senegal Drone Academy and certified by the national aviation authority, provides Congolese students with hands-on training and certification. It strengthens VillageReach’s model of sustainable, locally led innovation in health logistics and opens the door for broader drone applications across sectors.

The Integrated Laboratory Transport Systems to Stop Outbreaks program increases Access to Lab Services and strengthens epidemic preparedness in the Democratic Republic of Congo, Guinea, Malawi, Tanzania and Uganda. By enhancing sample transport networks, digitizing reporting systems, and training health workers and community leaders, the initiative accelerates the detection and response to diseases like polio, measles, Ebola and cholera. Real-time GPS and temperature monitoring ensure sample integrity, while community engagement fosters resilience, advancing universal health coverage across these nations.
