Malawi Faith Leaders Champion Adolescent Health Services: A Human-Centered Design Approach

In the rural community of Machinga District, Malawi, Father Justice Namangale of Mposa Anglican Church has emerged as a pivotal figure in promoting adolescent health initiatives, particularly the human papillomavirus (HPV) vaccine. As a respected faith leader, Father Namangale holds significant influence over the beliefs and practices of his congregation and the wider community in Mposa, Machinga. 

Father Namangale’s commitment to improving health outcomes has deeply resonated with residents. His respected position allows him to effectively communicate vital health information and dispel myths surrounding vaccinations. Through educational sermons and community programs, he has successfully raised awareness about the importance of the HPV vaccine in preventing cervical cancer, which significantly affects women’s health, including that of adolescents in Malawi. 

Recognizing the need to integrate health services for adolescents, VillageReach and Scope Impact have partnered with the Ministry of Health, with support from Gavi, The Vaccine Alliance to implement an innovative implementation study aimed at improving HPV vaccine coverage. This initiative focuses on exploring effective and sustainable approaches for integrating adolescent health services in Mchinji and Machinga districts. 

The HPV Integration Implementation Study workshop employed a Human-Centered Design (HCD) approach that enabled Father Namangale to create an environment where individuals felt safe to discuss their fears and questions. The workshop brought together key stakeholders, including faith leaders, health care workers, caregivers, teachers, community leaders, national health officials and donors. This collaboration has strengthened trust, which is now essential for effective communication regarding health interventions within Father Namangale’s congregation and the broader Machinga community.

Before engaging with the HPV Integration Implementation Study, Father Namangale observed a deep divide between health care providers and his community. Parents were reluctant to vaccinate their daughters due to fears that the vaccine would encourage early sexual activity. Many questioned whether the vaccine aligned with their faith and traditions. Father Namangale himself was uncertain about its purpose and struggled to provide clear answers when asked about it. Without accurate knowledge, he sometimes felt unprepared to guide his congregation in making informed decisions about their health. 

“I now understand that faith and science are not at odds—both can work together to protect our loved ones. The HPV vaccine is not about changing our values; it’s about safeguarding the future of our daughters. Seeing my congregation and the community embrace this knowledge has been one of the most rewarding experiences of my leadership.” 

With support from the program and armed with new knowledge, Father Namangale began incorporating HPV vaccine awareness into his sermons and community meetings. By openly addressing fears and misconceptions, he helped shift perceptions, leading more families in the community to seek HPV vaccination for their daughters. 

Father Namangale emphasizes the importance of conducting more HCD sessions across various districts in Malawi. These sessions foster meaningful dialogues among health care providers, parents, youth and community members to better understand their perceptions of adolescent health, including HPV vaccination. 

Engaging stakeholders is crucial for strengthening acceptance of the HPV vaccine through trust-building, education and tailored messaging that aligns with HCD principles. This example demonstrates how faith leadership can significantly enhance responsive primary health care and improve public health outcomes.
